INSURANCE DOCUMENT *

I am insured with valid third-party liability insurance with a minimum cover according to the event NOR or the equivalent. Proof of current insurance will be requested at registrations on site.

EVENT REGISTATION GENERAL DISCLOSURE *

I acknowledge that participation in this regatta is entirely at competitors own risk and no responsibility for damage injury or loss from participating in this regatta will rest with the Organising or National Authorities, Organising Committee, the Class Associations, the host Club, or any person or agent of these bodies. I declare that I am either the parent or other legal guardian of the above-named sailor or that I have been authorised by such a guardian to sign this document on their behalf.

RIGHT TO USE NAME AND LIKENESS *

In participating in this event any competitor automatically authorise the use of his/her personal data in compliance with Legislative Decree 196/03 and grants to the Organising Authority and the sponsors of the event the right in perpetuity, to make, use and show, from time to time and at their discretion, any motion pictures and live, taped or film television and other reproductions of him/her during the period of the competition for this event in which the competitor participates and in all material related to the said event without compensation.

PARENT/GUARDIAN DECLARATION

(REQUIRED FOR ALL SAILOR WHO ARE UNDER 18 YEARS OF AGE) As Parent/Guardian, I give permission for the participant named to take part in the event. I also confirm that my dependant is competent to take part in these activities and that the boat to be used will have valid third-party insurance of at least € 1.5m. I accept the Risk Statement in the Notice of Race which excludes the right to claim compensation in certain circumstances. I accept that good behavior of my dependent is a fundamental condition of participating and that the organisers have the right to exclude any sailor not complying with this requirement.
